Achieve STP Requirements With DESMOND!


Sustainability and Transformation Plans (STPs)

The NHS and local councils have come together in 44 areas covering all of England to develop proposals and make improvements to health and care. These proposals, called sustainability and transformation plans (STPs), are place-based and built around the needs of the local population.

Sustainability and Transformation Plans were initially set out in the NHS Shared Planning Guidance and the support implementation of the Five Year Forward View. They will be supported by the six national health and care bodies: NHS England, NHS Improvement, the Care Quality Commission (CQC), Health Education England (HEE), Public Health England (PHE) and the National Institute for Health and Care Excellence (NICE).

 

Diabetes teams across England have been allocated a share of the £44 million NHS England funding available to improve the treatment and care for people with diabetes.

CCGs have now applied for money from NHS England’s £44 million transformation fund. The cash has been earmarked to specifically improve the treatment and care for the 2.8 million adults and children diagnosed with Type 1 or Type 2 diabetes in the following ways:

 

Commissioning DESMOND means you are achieving your Sustainability and Transformation Plans (STP) requirements.

 

DESMOND is a NICE Quality Assured structured education programme for people with Type 2 Diabetes.

  

With the advent of the Diabetes STPs, the DESMOND programme helps deliver the successes required by 2020 through supporting people to manage their care effectively.

 

DESMOND as a programme can be delivered over one six-hour session or 2 half day sessions and when it comes to recording this on GP practice systems in readiness for the National Diabetes Audit the following codes can be applied:
