Localities: DESMOND sessions in Islington are held at Partnership Primary Care Centre (Camden Rd, London N7) and Bingfield Primary Care Cent
Referral Pathway: People are referred to DESMOND by their GPs or their Practice Nurses and other health care professionals. A completed referral form with requested biomedical data is faxed to the DESMOND Administrator at Partnership Primary Care Centre who will arrange attendence.

DESMOND Foundation module available(for those with established diabetes)
Islington has been running the DESMOND programme since 2006. The programme is mainly run over 2 afternoon sessions and there are some whole day sessions planned for 2009. In 2008, we piloted a couple of one -day Saturday sessions. They were very successful and in 2009, we are hoping to run more of them to accommodate those who are not able to attend the programme during the working week. We are also piloting our first BME Bengali DESMOND in March 2009, so watch the space.
DESMOND is run 3-4 times a month alternating between the 2 Primary Care Centres. With the increased number of programmes, NHS Islington funded training for more DESMOND Educators. We now have 10 in all and they include Dieticians, Diabetes Specialist Nurses, Podiatrist, District Nurse and Senior Nurses from the Practice Nursing Development Team.
