Local Referral Pathway: Referrals received from Health Care Professionals via referral form. Self-Referrals are also accepted currently via email / telephone as above.

DESMOND Newly Diagnosed (for patients diagnosed within the last 12 months)

DESMOND Foundation (for patients with established diabetes longer than 12 months)

Virtual DESMOND (For Newly Diagnosed and Foundation modules)

Overview

DESMOND Newly Diagnosed and Foundation are delivered face-to-face at one of 2 locations as below. Both locations offer free parking and have access to public transport links:

· Luton Treatment Centre, Vestry Close, Luton LU1 1AR

· Farley Hill Community Centre, Delphine Close, Luton, LU1 5RE

Virtual DESMOND is delivered in the comfort of your own home via Microsoft Teams

MyDESMOND is accessed individually via an App
