Localities: DESMOND is currently being delivered throughout Preston & Chorley . Face-to-face sessions take place at the Minerva Health Centre and Buckshaw Health Centre.
Local Referral Pathway: Referrals received from GP Practice, HCP, AHP and Self Referral
Modules Delivered:
DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)
DESMOND Foundation module available (for those with established diabetes)

Overview
We offer face-to-face sessions, virtual sessions, MyDESMOND and full day, half day and 2 hour DESMOND sessions. We also offer BAME DESMOND sessions.
