About Let’s Prevent Diabetes

Let’s Prevent Diabetes is a group education programme which aims to support people identified at high risk of Type 2 diabetes and work with them to prevent or delay the condition, through a structured education programme to target lifestyle modification and behaviour change which can be culturally adapted to suit your local population. What does this look like?

  • Six hours of structured self-management group education which can be delivered virtually or in-person - with an annual three-hour update called ‘Let’s Prevent Diabetes Going Forward’

  • Delivered by two trained Educators

  • Over one-day or two half days (or three 2-hour sessions if delivered virtually)

  • In groups of up to 10 participants who may wish to bring a partner/friend with them

  • Supported by specially developed resources

Content includes:

  • The risks and implications of developing Type 2 diabetes

  • Understanding diabetes

  • How to take control of risk: food choices and physical activity

  • How to make lifestyle changes to prevent its development

What can this do for your patients?

  • Improve their HbA1c

  • Improve LDL cholesterol

  • Reduces sedentary behaviour

  • Increases step count
