Overview

Anyone with Type 2 diabetes (new and existing) with a GP in the Barking & Dagenham area can be referred.

Once a referral is received you will be contacted by a DESMOND Co-ordinator to arrange your education day.

Courses are run twice a month at Porters Avenue Health Centre by a Diabetes Specialist Nurse and Registered Dietitian. We have a team of 7 Educators that deliver the courses.

The DESMOND session starts at 09:00 and finishes at 16:30. Refreshments for patients are provided.

We also offer MyDESMOND which is an online interactive digital programme consisting of 8 online sessions which you can access at your own pace.  It is an excellent opportunity to ‘Ask the Experts’ any questions you may have.  You need to contact the DESMOND Co-ordinator for access.

The DESMOND programme has proved to be extremely popular and successful and feedback from participants has been very positive.
