Local Referral Pathway: Referrals received from GP Practice and Practice Nurse. Calderdale GP referrals only. GP/Practice Nurse needs to complete a DESMOND referral form and send in the post to Diabetes Centre, The Calderdale Royal Hospital, Salterhebble, Halifax, West Yorkshire, HX3 0PW or email onto us at cah-tr.diabetesdietitians@nhs.net. Newly Diagnosed patients for type 2 diabetes are eligible for DESMOND course and need to be diagnosed within 12 months for us to accept referral.

DESMOND is an interactive group session; of up to 10 newly diagnosed Type 2 Diabetes patients, running over one full day (09:30 to 16:30) for face-to-face patients or two half days virtually via Microsoft Teams (09:30 to 12:30). The aim is to give participants the knowledge, ability, and confidence to start to manage their diabetes day to day.

Overview

Calderdale has had a DESMOND programme operating successfully since 2008. Courses are held in various localities all over the Calderdale area; covering Todmorden to Brighouse and everywhere in between. Please contact your GP or Practice Nurse for referral.

DESMOND is an interactive group session; of upto 10 newly diagnosed Type 2 Diabetes patients, running over one full day (09:00 to 12:00, 13:00 to 16:00) or two half days (09:00 to 12:00) and the aim is to give participants the knowledge, ability and confidence to start to manage their diabetes day to day.

All our DESMOND groups are facilitated by a Diabetes Dietitian and Podiatrist or Podiatry Assistant and we have a warm, friendly approach, with no pressure to participate on the day if you were thinking group sessions might not be for you. We welcome everyone along and encourage you to invite a supportive partner, relative or friend with you on the day; however due to limited space you may only invite one partner.
